Topic Overview
Key Takeaways
- Discontinuing antibiotics at discharge for complicated appendicitis does not increase post-operative infection rates
- No significant difference in deep organ space infections or SSIs between home antibiotic vs no antibiotic groups
- Protocol change safely eliminated home antibiotics in 121 complicated appendicitis patients without adverse outcomes
- Length of stay remained unchanged regardless of post-discharge antibiotic use
- Evidence supports antibiotic stewardship by avoiding unnecessary post-discharge antibiotics in complicated appendicitis

Do you give antibiotics after discharge in complicated appendicitis. Hi, I'm Cecilia Gigena from Cincinnati Children's Hospital, and I think this is an article that you should know about. This is a study done in Utah that wanted to compare patients before and after the implementation of their new protocol. This protocol consisted of suspending antibiotics at the moment of discharge in patients with complicated appendicitis. They had 185 patients in the home antibiotic group and 121 patients in the no home antibiotic group, and they found that there were no significant difference in deep organ space infections, SSIs, or length of stay. So it seems that we can safely discharge our patients with complicated appendicitis without antibiotics. Let us know what you think and stay tuned for more articles that you should know about.
