Topic Overview
Key Takeaways
- Transcystic laparoscopic CBD exploration had significantly lower complication rates than ERCP-first approach in pediatric choledocholithiasis
- Transcystic laparoscopic approach resulted in shorter hospital length of stay compared to ERCP in this multi-center US study (2018-2022)
- Multi-center data (252 patients) suggests attempting transcystic laparoscopic CBD exploration first may benefit pediatric patients with choledocholithiasis
- ERCP-first strategy may not be optimal for all pediatric patients with suspected common bile duct stones based on comparative outcomes

Do you only use ERCP for clinical lithiasis? Hi, I'm Cecilia Higuera from Cincinnati Children's Hospital, and I think this is an article that you should know about. This is a multi-center retrospective study done in the US from 2018 to 2022, and their aim was to compare patients with suspected clinical lithiasis that underwent a transcystic laparoscopic common bile duct exploration versus those who underwent ERCP first. They gathered 252 patients, 156 in group one, the transcystic laparoscopic common bile duct exploration, and 96 in group two, patients who underwent an ERCP first. And what they found is that patients in group one had significantly less complication rates and lower length of stay. So it seems that attempting a transcystic laparoscopic common bile duct exploration may benefit our patients. Let us know what you think, and stay tuned for more articles that you should know about.
